You must bring a completed lab slip and photocopy of your drivers license/ID card and a photocopy of the front and back of your insurance card (if applicable). If you are getting the CardioMetabolic, CMP or BMP tests you must be fasting for 10-12 hrs prior. You are encouraged to drink plenty of water.
